ConnectCare Medical Clinic was established by Physicians for Physicians who aim to create a true Patient-Centred Medical Home through a comprehensive Collaborative Care model. As a team, we work together for the mutual benefit of our patients and to create a positive environment for all staff, nurses, MOA’s and physicians. And, unlike a traditional medical clinic, ConnectCare has the advantage of an in-house full service Pharmacy and Pharmacist all in the same location. This further extends our collaborative care model further than ever before. From prevention to treatment to medication, our patients are provided the convenience they want with the optimal care they deserve.
